Use ARRAY_SIZE macro instead of sizeof(x)/sizeof(x[0]) and remove
duplicates of ARRAY_SIZE.  Some coding style and trailing whitespaces are
also fixed.

Compile-tested where possible (some are other arch or BROKEN)
